1. Long Form Certified Birth
Certificate or United States Naturalization
Birth certificate must be legible and the
seal is noticeable and raised. No taped certificates
will be accepted. No hospital certificates or an
acknowledgment certified by the clerk that a birth
certificate is filed in their office.
If you were born
in Brownsville you can obtain a copy of your birth
certificate at the City Secretary's Office located at 1034
E. Levee St., Brownsville, Texas. 548-6054.
Certified copies are $24.00.

2. Current Government Issued
Photo Identification (Driver's License Texas ID)
Applicants who are 18 years
are older must have ID
3. All applicants need to be present,
including children.
Children under 16: Both
parents must be present with child. If only one parent
is available, form DS-3053 Statement of Consent-Issuance of
a Passport to a Minor Under Age 14 is required. One
parent must be present with child.
4. Check/Money Orders
Separate fees will be required per
application; therefore multiple checks or money orders will
be needed. Example: 2 checks or money orders per
application - if a parent and child are applying 4 checks
will be needed.
5. Two 2"x2" photos
Photos must be on white or
off-white background, well focused, on high quality photo
paper and the size of their face must fall within the
guidelines on the application. Photos will be taken on
site for $15.00.
6. Do not sign
application.
White out cannot be used on
application.
